Dynasty Studio of Dance Class and Financial Agreement Contract
Dynasty studio of Dance operates on a seasonal basis meaning tuition is paid in monthly installments or in full for an entire season. Monthly installment prices depends on the classes your child is registered for. Understand that if you register your child for any classes during a season you are financially responsible for all installment payments for that season even if your child decides to leave the studio. Everyone will be given a 30 day grace period from the date of registration to opt out of the agreement with a $50 fee. All monthly payments are due before or on the 2nd Friday of each month. A late fee of $15 will be added to your child’s account if not paid by then. After a week, if no payment is received your child will not be able to attend class until account balance is settled.
Please initial for the following statements and sign at the bottom:
• I understand that I will be held financially responsible for all monthly installments due every 2nd Friday of the month for the 21-22 dance season
• I understand that if my child decides to leave the class for any reason during the season that I am still financially responsible for the remaining installments due for that season
• I understand that I have a 30 day grace period to opt out of this financial agreement and the class with a $50.00 fee associated
• I understand that a late fee will be applied to my child’s account if I do not make a payment by the 2nd Friday of each month
After reading and signing this contract, I fully understand and agree to all terms listed above. By signing this I will be making myself financially responsible for all monthly installments due to Dynasty Studio of Dance For the 2021-22 Dance season (Dec.2020-June2022).
