VINE CITY SPORTS & ALL STARS — AUTO-PAY AUTHORIZATION

By enrolling at Vine City Sports & All Stars, I acknowledge and authorize the following Auto-Pay terms:

I authorize Vine City Sports & All Stars to automatically draft all tuition, fees, costume/uniform fees, performance fees, and any other charges related to my account using the payment method I have provided in my Studio Pro Parent Portal.

Monthly tuition is drafted on the 1st of each month for the upcoming month’s classes. If the 1st falls on a weekend or holiday, tuition will be drafted the business day before.

I am responsible for ensuring that valid and current payment information is saved in my Parent Portal at all times to allow for successful processing.

If payment is not successfully processed by the 5th of the month, a $35 late/reprocessing fee will automatically be applied to my account.

If my account remains unpaid for more than 10 days, my child may be removed from classes until the account balance is paid in full.

I understand that there are no refunds or credits for missed classes, holidays, or weather-related cancellations.

If I wish to withdraw from classes, I must submit written notice to info@vinecitysports.com prior to the next month’s billing cycle. Withdrawals requested after tuition has been drafted will apply to the following month.

By agreeing to this authorization, I confirm that I have read, understand, and accept the Auto-Pay policies, and I consent to all scheduled and authorized payments.

VINE CITY SPORTS
FACILITY RENTAL AGREEMENT & LIABILITY WAIVER

This Agreement is entered into by and between Vine City Sports, LLC (“Vine City”) and The Block, LLC located at 1040 Mustang Drive Grapevine, Texas 76051, and the undersigned Renter (the “Coach/Instructor/Renter”), for the use of designated studio, field, or training space (the “Facility”) for training, instruction, classes, or other events.

1. FACILITY USE
The Renter agrees to use the space only for the purpose agreed upon with the owner [e.g., Music lessons, dance instruction, sports training, private lessons].

The Renter shall have access only to the space agreed upon by Vine City. Use of any additional areas (equipment, storage, office, etc.) must be approved in advance.

All setup and teardown must occur within the reserved rental period.

2. RENTER RESPONSIBILITIES
By signing this agreement, the Renter agrees to:

Be fully and solely responsible for all participants, guests, and any assistants under their supervision.

Ensure all participants are informed of the nature of the activity and any risks involved.

Return the space to its original condition. This includes cleaning up any trash, spills, or equipment after use.

Immediately report any damage or incident occurring during the rental period.

The Renter assumes all financial responsibility for damages caused during the rental period, whether accidental or intentional, by themselves, their staff, or their participants.

3. INSURANCE REQUIREMENTS
Vine City strongly recommends that Renters carry general liability insurance.

Vine City may, at its discretion, request a Certificate of Insurance (COI) naming Vine City Sports, LLC as an additional insured.

4. LIABILITY WAIVER & HOLD HARMLESS AGREEMENT
The Renter acknowledges and agrees to the following:

Vine City Sports, LLC, its owners, agents, employees, and affiliates shall not be held liable for any injury, loss, damage, or death resulting from the Renter’s activities or use of the Facility.

Renter releases, waives, discharges, and covenants not to sue Vine City or its owners and The Block or its owners for any liability, claims, demands, or causes of action arising out of or related to the use of the Facility.

This waiver includes, but is not limited to, injuries that may occur as a result of equipment malfunction or misuse, slipping or falling, or any negligent act or omission of Vine City or others.

5. CANCELLATION & REFUND POLICY
Cancellations made more than 24 hours before the scheduled time may receive a full refund or reschedule credit.

No refunds will be issued for cancellations made within 24 hours of the scheduled rental.

Vine City reserves the right to cancel any rental at any time. A full refund will be issued in such cases.

6. RULES & EXPECTATIONS
Only clean, non-marking athletic shoes or dance shoes are allowed in activity spaces.

No guns, tobacco, smoking, vaping, alcohol, or open flame is permitted anywhere on the premises.

Children not participating in the rental activity must be supervised by an adult at all times.

Music and noise levels must be kept at a respectful volume, especially during concurrent rentals.

7. GENERAL
This Agreement represents the entire understanding between the parties and supersedes all prior negotiations or agreements.

This Agreement shall be governed by the laws of the State of Texas.

Renter Acknowledgment

By submitting a rental request, I acknowledge that I have read, understood, and agreed to all terms outlined in the Facility Rental Agreement and Liability Waiver. I accept full responsibility for the conduct, safety, and supervision of all individuals participating under my care during the rental period.

VINE CITY SPORTS & ALL STARS — LIABILITY WAIVER & RELEASE OF CLAIMS

By enrolling myself or my child(ren) at Vine City Sports LLC ("Vine City") or The Block, LLC ("The Block"), I acknowledge and agree to the following terms and conditions:

Assumption of Risk

I understand and acknowledge that participation in dance, tumbling, cheer, fitness, strength, agility, and all physical activities offered by Vine City involves inherent risks, including but not limited to falls, injuries, accidents, illness, permanent disability, or death. I voluntarily assume all risks related to my child’s (or my own) participation, whether caused by myself, my child, instructors, other participants, or equipment.

Medical Authorization

In the event of injury or illness, I authorize Vine City staff to obtain any necessary emergency medical treatment for my child or myself. I agree to be financially responsible for any medical care provided.

Release of Liability

I release, waive, discharge, and hold harmless The Block, LLC and Vine City Sports, LLC, its owners, directors, officers, employees, instructors, contractors, volunteers, and facility owners (collectively, the “Releasees”) from any and all claims, demands, causes of action, losses, or liabilities for personal injury, property damage, or wrongful death arising from participation in any Vine City program, class, practice, event, or activity, whether caused by the negligence of Vine City, its staff, or otherwise.

Indemnification

I agree to indemnify and hold harmless Vine City and the Releasees from any claims, demands, or costs (including attorney’s fees) arising out of my child’s or my own participation, including claims brought by other participants, family members, or third parties.

Financial Responsibility

I acknowledge that I am financially responsible for all tuition, fees, and charges associated with my child’s enrollment and participation, according to Vine City’s published policies and procedures.

Photography & Media

I understand that Vine City may photograph or record participants for marketing, educational, or promotional purposes. I grant permission for my child’s (or my own) likeness to be used as outlined in the Photo & Media Release.

Severability

If any portion of this agreement is found to be invalid or unenforceable, the remaining portions shall remain in full force and effect.

Acknowledgment of Policies

I acknowledge that I have read and agree to comply with Vine City Sports & All Stars’ Policies & Procedures, Privacy Policy, Auto-Pay Authorization, and all additional studio policies.

Legal Capacity

I am the parent or legal guardian of the registered participant(s), or I am registering myself, and I have full legal authority to enter into this agreement.

By completing registration, I confirm that I have read, understand, and voluntarily agree to all terms in this Liability Waiver & Release of Claims.

VINE CITY SPORTS & ALL STARS – POLICIES & PROCEDURES AGREEMENT
By enrolling my athlete(s) at Vine City Sports & All Stars, I acknowledge and agree to the following policies:

ATTENDANCE POLICY

Attendance is taken at every class.
No refunds, credits, or prorated tuition for missed classes.
Make-up classes may be scheduled in the same month in a similar class.
Missed classes due to injury may be made up within 1 month after receiving medical clearance.
More than 3 unexcused absences may result in changes to choreography, level placement, or removal from class.

Excused Absences Include:
Fever (100.4+), contagious illness, first 24 hours on antibiotics, family emergencies, graded school functions. Doctor’s note required when applicable.

Mandatory Events:
Performance participation is expected unless discussed at registration. Missed performances may result in removal from routines.

DRESS CODE

Ballet:
Girls – Vine City approved leotard, pink tights, pink ballet shoes, hair in bun.
Boys – Black shorts, white t-shirt, black ballet shoes.

All Other Classes:
Girls – Fitted dance top and bottoms, appropriate shoes, hair pulled back.
Boys – Athletic shorts, t-shirt, appropriate shoes.

Failure to wear proper attire may result in being asked to sit out.

PARENT EXPECTATIONS

Support the chain of communication: Coach → Director → Owner.
Model respectful conflict resolution. Avoid venting to other parents or athletes.
Commit to the full session (Summer: June–July, Fall/Spring: August–May).
Respect all staff, parents, and athletes in person and online.
Negative comments about Vine City (including on social media or group chats) may result in dismissal.
Arrive on time for events and ensure athletes arrive “performance ready.”
Attend required parent meetings and respond to BAND action items within 48 hours.
Stay current on all financial obligations.
Failure to follow parent code of conduct may result in athlete probation or removal.

ATHLETE EXPECTATIONS

Respect coaches, classmates, staff, and facilities.
Zero tolerance for bullying (in-person or online).
Represent Vine City with integrity at all times.
No foul language, immoral behavior, or inappropriate conduct.
Arrive prepared, on time, and ready to work.
Wear proper class attire or sit out.
Be available for meetings if requested by staff.
Make safe choices for self and others.

TUITION POLICY

Tuition is drafted monthly on the 1st via Studio Pro Auto-Pay.
If the 1st falls on a weekend/holiday, draft will occur the business day before.
$35 late fee applies if payment is not received by the 5th.
After 10 days unpaid, athletes may be removed from classes.
No refunds, credits, or prorated tuition for missed classes, holidays, or weather closures.
Withdrawal requires written notice emailed to info@vinecitysports.com before the next billing cycle.

REGISTRATION FEE

A $50 non-refundable registration fee per family is due at enrollment.
Enrollment is not complete until online registration, payment info, and tuition are submitted.

DROP OFF & PICK UP

Athletes may arrive no earlier than 15 minutes before class.
Ensure a coach is present before dropping off.
No waiting outside for rides. Notify the front desk if your athlete leaves with another student.

SAFETY & PROPERTY

Vine City staff is working to become USASF certified and committed to age-appropriate material.
Vine City is not responsible for lost, stolen, or damaged property.
Athletes are expected to keep facilities clean and leave spaces better than they found them.
Vandalism will result in immediate dismissal.

SOCIAL MEDIA

Conduct on social media must reflect Vine City values.
No posting photos/videos of other athletes without consent.
Phones are discouraged during class.
Negative social media activity may result in removal.

PERFORMANCE TEAM PARTICIPATION

Performance opportunities are available only to dancers enrolled in a Performance Team or All Star Team program. Recreational classes do not include performance participation. There will not be a studio-wide end-of-year recital. Performance schedules and event details will be announced and updated in BAND throughout the season.

INTELLECTUAL PROPERTY

All choreography, logos, costumes, curriculum, and policies are property of Vine City Sports & All Stars may not be reproduced or distributed without written permission.

AGREEMENT ACKNOWLEDGEMENT

By completing registration, I acknowledge that I have read, understand, and agree to all Vine City Sports & All Stars policies. I accept full responsibility for my athlete(s) and my own compliance with these policies throughout the season.

VINE CITY SPORTS & ALL STARS — PRIVACY POLICY

By registering with Vine City Sports & All Stars ("Vine City," "we," "our," or "us"), you acknowledge and agree to the collection, use, and protection of your personal information as outlined below:

YOUR PRIVACY CHOICES

You may update your account information through your Studio Pro Parent Portal or by contacting us at info@vinecitysports.com.

You may opt out of marketing emails by following the unsubscribe instructions provided in those emails.

CHILDREN'S PRIVACY

We do not knowingly collect information from children under 13 without verified parental consent. If you believe we have collected such information in error, contact us immediately so we can remove it.

DATA SECURITY

We use reasonable administrative and technical safeguards to protect your personal information. However, no system is 100% secure, and you share information at your own risk.

THIRD-PARTY LINKS

Our website and portal may contain links to third-party sites. We are not responsible for their privacy practices. Please review third-party privacy policies before providing any information.

INTERNATIONAL VISITORS

By using our services, you consent to the transfer of your information to the United States, where Vine City operates.

VINE CITY SPORTS & ALL STARS — PHOTO & MEDIA RELEASE

By enrolling in Vine City Sports & All Stars programs, I (the parent/guardian) grant permission for Vine City Sports & All Stars, its staff, and authorized representatives to photograph, record, and/or videotape my child(ren) during classes, events, performances, and other studio-related activities.

I understand and agree that:

Photographs, video, and/or audio recordings may be used by Vine City Sports & All Stars for promotional, marketing, educational, or informational purposes.

Media may be used in printed materials, brochures, flyers, website content, social media platforms, advertisements, and other official studio communications.

Vine City Sports & All Stars will not sell or license any photographs or recordings to third-party companies for commercial use.

No full names, identifying information, or contact information will be shared publicly without additional written consent.

Participation is voluntary, and no compensation will be provided for the use of such images or recordings.

I understand that I may revoke this consent at any time by submitting written notice to info@vinecitysports.com. If revoked, Vine City will make reasonable efforts to discontinue the future use of my child’s image but may not be able to remove previously published materials.
