SDCA SAFE DANCE & CHEER PRACTICE POLICY
I understand that my child will participate in dance and/or cheer training at SDCA. While all reasonable care is taken and safe teaching practices are followed, I acknowledge that physical activity carries an inherent risk of injury and that accidents may occur.
I consent to first aid being administered to my child in the event of an injury or emergency. If I am unable to be contacted, I authorise SDCA to arrange emergency medical treatment, including transportation by ambulance if deemed necessary.
I agree to notify SDCA of any medical conditions, illnesses, or injuries sustained outside of classes that may affect my child’s ability to participate safely, so appropriate care and modifications can be made where required.

MEDIA & PHOTOGRAPHY CONSENT
I acknowledge that classes, rehearsals, and events may be photographed or filmed from time to time for educational, training, or promotional purposes.
I consent to SDCA using images and video footage of my child for:
Technique review and teaching purposes
Studio promotions, marketing, and social media
SDCA website and related advertising materials
If I have specific concerns regarding media use, I understand that these must be submitted in writing to SDCA administration.

PAYMENT & FEES POLICY
I understand that class fees are payable monthly via SDCA’s secure online payment portal.
Additional fees may apply depending on enrolment, including but not limited to:
Costume fees
Performance or competition entry fees (where applicable)
I acknowledge that an annual enrolment fee of $20 per child applies, which covers student insurance and administration costs.

OUTSTANDING FEES & WITHDRAWALS
I understand that all fees are non-refundable and non-transferable.
In the event that an account becomes overdue and requires referral to a debt collection agency, I acknowledge that the parent or guardian listed on the account will be responsible for all associated collection costs. Any discounts applied to the account may be forfeited.
If I wish to discontinue classes, I understand that written notice must be provided to SDCA via email. Fees will continue to be charged until the end of the current monthly payment cycle.

ACKNOWLEDGEMENT & AGREEMENT
I confirm that the information provided is accurate and up to date. I have read, understood, and agree to the terms outlined in the SDCA Safe Dance & Cheer Practice Policy, Media & Photography Consent, and Payment Policies.
I acknowledge that SDCA has established Student and Parent Codes of Conduct designed to protect the safety, wellbeing, and enjoyment of all SDCA families and staff, and I agree to adhere to these expectations.
Copies of the SDCA Codes of Conduct are available at the studio and within the parent portal. Families are encouraged to familiarise themselves with these policies.
