________ TUITION Monthly payments are due by the first scheduled class of the month, and a $25 late fee is applied to overdue accounts after the 5th of each month.
________ PHOTO RELEASE: The school is hereby granted permission to take photographs and videos of students to use in brochures, web sites, posters, advertisements and other promotional materials the school creates. Permission is also hereby granted for the school to copyright such photographs in its name.
________ RISK OF DANCE: I understand dance and related activities involve risk of injury. I agree I will not hold San Diego Hula Academy nor Momentum Training Center (dance facility). (its owners, staff and related parties) responsible for injuries/damages incurred by any of my family members while participating or visiting the facility.

REGISTRATION:
San Diego Hula Academy (SDHA) has periodic registration periods. The greatest growth and progress occur when students stay in the same class with the same students for the academic year, just like in school. (Students can always enroll mid-year if there is room in a class.)

A $50 registration fee is due once per year per student billed each January. A maximum of $100 will be billed per family for any families with more than one dancer.

Registration at San Diego Hula Academy automatically includes you in the special events, workshops and performances.

TUITION:

Tuition rates are based on the length of the entire academic year, so tuition installments are the same whether there are 1, 2, 3, 4 or 5 classes within the month. Tuition does not change based on attendance. (If registering mid-month, then your first automatic payment will be prorated to reflect your start date.)

Tuition is per month not per class. Multi-family discounts are available for members of the same household. Senior and military discounts are also available.

Tuition may be paid by the following methods: cash, check, Zelle, Venmo, or credit card (We accept MasterCard, Visa, and Discover).

The parent or guardian should notify San Diego Hula Academy of any change to the credit card or checking account. The returned check/declined card fee is $35.

All fees paid are nonrefundable and nontransferable.

CANCELLATION. There is no long-term contract, so just call 858-243-5071 or stop by 10 days before your billing date to stop future payments.

HĀLAU INFORMATION & DRESS CODE:

PRACTICE ATTIRE
Hair should always be securely pulled away from the face to not distract the dancers during class time.

For hula, all students should wear a hālau shirt and paʻu skirts (for female students) and lawa lawa (for male students) are required. Shorts under paʻu skirts for female students are necessary for modesty. Class uniforms are required by the 3rd month of enrollment.

For Tahitian, students are encouraged to wear form-fitting dance attire to display proper body alignment and allow for easy movement. Pareo/sarong are also requested. We will have some pareo on site for those that need to borrow. Class uniforms are required by the 3rd month of enrollment.

RISK OF DANCE
I agree that I will not hold San Diego Hula Academy or any faculty member, or employee of the same, liable for injuries sustained or illness contracted by me while a student of San Diego Hula Academy.

CLASS CANCELLATIONS
In the unlikely event of a class cancellation as a result of adverse weather conditions, dance class cancellations will be posted on our website home page, Facebook, and Instagram. You may also call the school at 858-243-5071 for current information.

________ RISK OF DANCE: I understand and agree to the nature of the classes and performances offered by SDHA; that the student is qualified and in good health, and in proper physical condition to participate.
I understand dance and related activities involve risk of injury. I knowingly and voluntarily, accept and assume responsibility for each of the risks and dangers, and all other dangers that could arise out of student’s participation in classes or performances.
I agree I will not hold San Diego Hula Academy, nor Momentum Training Center (dance facility), (its owners, staff and related parties, and all affiliated entities (hereinafter collectively referred to as “SDHA”) responsible for injuries/damages incurred by any students while participating or visiting the facility.
________ TERMINATION: The undersigned may terminate this agreement at any time by giving written notice to SDHA in person or at the following address: San Diego Hula Academy, 1740 La Costa Meadows Drive, Suite 250, San Marcos, CA 92078. Balances remaining on accounts must be paid upon departure. No additional fees for termination will be applied.
I have carefully read this Agreement, including paragraphs regarding termination, risk, release, and have the opportunity to ask questions about the same. I understand that this is an assumption of risk and release of liability, and I sign it of my own free will.
________ PHOTO RELEASE: I also authorize SDHA to take photographs and videos of my child/myself/the minor for whom I am legal guardian for promotional purposes. The use of images include, but are not limited to, brochures, web sites, posters, advertisements and other promotional materials SDHA creates. Permission is also hereby granted for the school to copyright such photographs in its name.
