*ATTENTION* CHANGES HAVE BEEN MADE TO 2024-2025 WAIVERS.
Please read carefully and sign that you have reviewed and agree to the following policies to secure registration at Prestige Performing Arts (PPA).
Please note that all accounts and fees due can be seen on the parent portal that you have created.
Dance/Acro/& or Acting for Camera classes:
1. I understand that tuition is a yearly price, split up into 10 monthly installments for convenience. This means that the same monthly payment is due regardless of the number of classes in a particular month.
2. I understand that a Registration Fee ($30 per child or $40 per family) and First Month's tuition is due upon enrollment. The last 2 installments of May and June tuition will be due on May 1st.
3. I understand that it is required to have an active credit card on file.
4. I understand tuition will be billed to the portal on the 25th of the preceding month and is due by the 1st. To avoid additional credit card processing fees you can pay via Cash, Check or Zelle. If payment is not made BEFORE the 1st of the month, your credit card on file will be charged the next day.

5. I understand that if a credit card is declined an automatic email will be sent to me. I understand if I do not give further instructions the card will be charged again on the following day. I understand that if it is declined for the second time another email will be sent and an additional $20 nonrefundable late fee will be charged to my account. This fee continues monthly until the tuition is paid in full. I understand making payments on time will avoid accumulating any additional charges.
6. I understand that there will be a $35 fee for all bounced checks.
7. I understand that classes are not prorated, and that all fees such as registration, tuition, costumes, jewelry, etc. paid to PPA are NONREFUNDABLE.
8. I understand that PPA instructors reserve the right to dismiss any student from class who is continuously disruptive towards teachers or peers. All students are expected to behave in a respectful manner.
9. I understand PPA has a required dress code to attend class.
10. PPA will communicate upcoming events and deadlines via email. I understand that it is my responsibility to read the emails for important updated information.
11. I understand as a parent/guardian of a PPA student I grant permission for photos/videos of my registered child to be used to promote its studio on their website, Facebook, Instagram, Social Media pages etc.
12. I am aware that my child's cell phone/valuables should be placed safely in a cubby during class and that PPA will not be held accountable for any lost or missing items.
13. I am aware that my child may not use or check their cell phone during class.
14. I am aware that there is a cancellation policy in place for all private lessons. PPA requires 24 hours notice for a private lesson cancellation. (Please keep in mind that not only is space set aside for the individual, but the instructor has set aside time and travel for that lesson). If less than 24 hour cancellation notice is given PPA will be compensated for the cost of the lesson in its entirety.
15. I understand that there will be a mandatory $30 Recital Video Fee billed to each family's account on April 1st. This guarantees a digital download for all Recital shows for each family. This fee only applies to Recital & Competition Families.
16. I UNDERSTAND CLASSES ARE NOT PRORATED. IF A CHILD WILL NOT BE ABLE TO ATTEND THE FULL MONTH, HE/SHE CAN ATTEND A MAKEUP CLASS OF THEIR CHOICE. ALL MAKEUPS MUST BE DONE WITHIN THE CURRENT SEASON.
17. I UNDERSTAND IF A CHILD WITHDRAWS FROM A CLASS A WRITTEN NOTICE VIA EMAIL PRIOR TO THE NEXT BILLING CYCLE IS REQUIRED. IF A CLASS IS DROPPED ON THE FIRST OF THE MONTH OR AFTER I WILL BE RESPONSIBLE FOR PAYING THAT CURRENT MONTH.
I assume all financial responsibility for the above student enrolled at Prestige Performing Arts. I have read and will follow all rules and policies of PPA.

For Theater/Musical Productions:
