RISE Dance Studio/RISE Preschool/Rise Tumbling - WAIVER AND RELEASE OF LIABILITY FORM

My child and or children, hereby agree to the following:

That I am participating in the training, dance, tumbling, preschool, fitness and events at RISE.
In consideration of participating in any classes, I agree to assume full responsibility for any risks, injuries or damages, known or unknown, which I might incur as a result of participating in the class.

I hereby authorize RISE staff to act on my behalf, in accordance with his or her best judgment, in case of an emergency involving my child and agree to assume full responsibility for all expenses, medical or otherwise, that may arise as a result of injury or accident sustained while participation in Little Valley Dance, LLC programs.

I, my heirs or legal representatives forever release, waive, discharge, and covenant not to sue RISE in event of any injury caused by their negligence and or other acts.

Photo Release. I grant full permission for organizers/teachers to use photographs, portraits, films and videos of my child made by him/her in legitimate accounts and promotions of the class and RISE.

All students who attend classes at RISE take full responsibility for their own safety and health and come with the knowledge of the risk and potential exposure involved and agree to accept any and all inherent risks to their personal health. By coming to the studio you should be symptom-free and you should also not come to the event if you are known to have been exposed to someone with COVID-19. RISE is not liable, attend at your own risk.

All accounts are required to enroll in Auto Payment each month. Accounts will be charged on the 1st of the month for tuition and the due date for other fees.

There is no fee for those enrolled in auto-pay with ACH (checking account).
There is a 3.50% + $.30 fee for those enrolled in auto-pay with a credit or debit card.

I understand that there are no prorated months throughout the season.

If a student would like to drop from a class they need to contact RISE and be removed from class. If you would like to drop from a class you need to contact RISE and be removed from class. I agree to the 30-day cancelation policy.
I must give a 30-day notice before dropping a class, I will not receive a refund tuition for dropped classes after the month has begun. Payments will continue to be charged until the student is removed from class. You must receive an email confirming that your student has been dropped from the class.

If tuition and fees are not paid for 2 months student will be removed from class and charges for missed payments will be required.

I have read the above release and waiver of liability and fully understand its contents. I voluntarily agree to the terms and conditions stated above.
