I understand that anyone is welcome to participate at The Attic providing: they have not been removed from another program for conduct, they do not mock the values/beliefs of others or The Attic, and they are not harmful to others (physically or with their words).
I recognize and acknowledge that there are risks of physical injury and of contracting a contagious illness/virus to participants in any program/activity and I agree to assume the full risk of any injury, illness, damage, and/or loss regardless of the severity in which I or my child/children participate or attends (on site and off) and including when my child is on premise between/before/after activities or traveling to and from activities/events. I fully release, waive, hold harmless and discharge The Attic and all ownership and personnel associated with The Attic including all instructors/contractors from any anticipated or unanticipated injury, illness, damage, and/or loss regardless of the severity that occurs on or about the premises of The Attic as well as any off-site activities (including any traveling to and from activities/events). It is always recommended to consult a physician before undertaking any physical activity.
I give permission for photos or video to be taken during activities/classes at The Attic (as well as off-site activities coordinated by The Attic) for publication and use as The Attic should decide. If you do not wish for you/your child to be photographed or videotaped (will not be able to be in any practice videos, etc), you must check the box that photographs should not be taken of your child.
During the Sept-May program, I understand the registration system requires 10% down to reserve a spot. The other 90% will post to my account 10% at a time (at the beginning of each month) for the 9 months (Sept-May). If I want to hold a spot, but not pay at this time, I will make my selections and when I get to the payment screen, I will email info@theatticdance.com and they will override and post the 10% to my account to pay by August 31st. From the parent portal, if I choose to, I can set up an autopay that will pay as tuition is billed.
Summer programs are pre-pay with no discounting, refunds, or credits for missed classes. Summer programs all show as registration fees.
I understand that if my account is more than 60 days past due, a late fee of $10 will be charged and then each month thereafter until brought current. I understand if a check is returned, a $30 fee will be charged to my account.
