Amateur Minor Athletic Waiver and Release of Liability
In consideration of being allowed to participate in any way in Linn County Dance & Cheer LLC Dance, Cheer, or Tumbling/ programs, and related events and activities, the undersigned:

1. Acknowledges and fully understands that each participant will be engaging in activities that involve risk of serious injury.

2. Assumes all the forgoing risks and accepts personal responsibility for the damages followed by such injury.

3. Releases, waives, discharges and covenants not to sue Linn County Dance and Cheer LLC, or USD #346, its affiliated clubs, their respective administrators, directors, coaches, and other employees of the organization, and if applicable, owners and lesser of premises used to conduct the event, all of which are hereinafter referred to as “releases” from any and all liability to cache of the undersigned, her or his heirs and next of kin for any and all claims, demands, losses, or damages on account of injury, caused or alleged to be caused in whole or in part by the negligence of the releases or otherwise.

Parent Information

Parents we understand that this is a time-consuming, and at times an expensive sport. The sport of dance and cheer takes a total commitment by the entire family. Just as we have rules and regulation for the dance and cheer members, there are certain rules that we ask of or parents and families.

- Parents & spectators are not allowed in classes or practices unless invited by the staff.
- Parents are allowed to record and take photos in class of their children only! If we see you taking pictures of other children, we will ask you to leave.
- Name calling, disrespecting other parents or children, and down talking a coach are not permitted.
- Please be respectful of our practice times. We are always happy to listen to any new ideas you feel may be helpful to our program. During practice or a performance is not the appropriate time for these discussions though. Please find time prior to or after practices. Parents may also find emailing a useful way to resolve minor issues.
- Please drop off and pick you child up for their scheduled class times, if you run late, we run late.
- Do not bring a sick child to class.
- Payments are due on the 1st of each month. There will be a $5.00 late charge for any payment not received by the 10th of the month.
- appropriate dance, cheer, and tumbling attire is required for all classes. Appropriate shoes for each class are also required.
- Students are expected to make it to classes. When they miss classes they fall behind. If a student misses too many classes we will ask that they start again next season but will be done for the current season.
- If students miss a class, it is their responsibility to learn what we taught. We cannot spend valuable class time reteaching old lessons.

As a parent your support is crucial, without you there are no kids, and thus no program. The Studio will not show favoritism to any child. We feel that every child is an important part of the entire program. Thank you for choosing to be part of Linn County Dance & Cheer Academy’s Dance/Cheer program, we are thrilled to have the chance to work with your child.

General Rules & Regulations
• There will be NO food, gum, or drinks on the dance/cheer floor. You may bring a bottle of water for breaks, but it must stay behind the bar area.
• Back talk, rolling of the eyes and any other disrespectful gestures toward coaches will not be tolerated. After a warning, the member will be asked to call a parent, and arrange a meeting to discuss the problem. The next offense that happens will result in the member being removed from the studio.
• The Studio is a Drug and Alcohol free environment. Abuse of this policy will require immediate removal from the team.
Every student should make a habit of being in class and practicing outside of class. If a student misses more than 25% of classes, they will be ineligible for recital. This is to make classes worthwhile for all. When one student misses continuously it affects all in that class.
• Teammates/classmates are expected to treat one another with mutual respect. They should talk to one another in a manner in which they wish to be spoken to. We do not demand that you embrace other member as your best friend, but pettiness, gossiping, or cliques, which attempt to exclude certain members, will not be tolerated.
• Any negative use of social media, such as Twitter or Facebook, can and will result in removal from the team/class.
• Excessive tardiness/absences will result in removal from the team/class. It is disruptive to the rest of the team/class.
• We understand that there are circumstances for which absences are unavoidable; however, this is an activity that you have chosen to participate in. Your attendance not only affects you, but all the other members of the team/class.
• Proper dance/cheer/tumbling attire and shoes are required.
• No Jewelry will be worn in classes.
• No cell phones are allowed in practice unless we ask you to record something we just taught.
• If you are sick or have been running a fever you are not to attend class.
• ABSOLUTELY no stealing!!! This will result in automatic and permanent expulsion.
