Liability Waiver, Photo Release, and Acknowledgment of Risk

REGISTRATION IS INCOMPLETE WITHOUT CONSENT
AND MUST BE GRANTED BEFORE ATTENDING CLASS
I understand and agree that in participating in any class, workshop, rehearsal, performance, camp, or activity there is a possibility of physical injury and/or death. I voluntarily agree, therefore, to assume all risks and responsibility for any such injury or accident, which might occur to me or my children during any Shall We Dance classes, rehearsals, performances, camps, or activities, regardless of whether the class, rehearsal, performance, camp, or activity takes place within the actual confines of the Shall We Dance studios, gym, or elsewhere. I also exempt, release, and indemnify West Virginia Ballroom, LLC, and Shall We Dance Studios, its Summersville Gymnastics Center, its owners, agents, staff, volunteers, assistants, employees, guest artists, faculty members, teachers, teacher assistants, instructors, coaches, spotters, and/or students as well as Ninja Zone from any and all liability claims, demands, or causes of action whatsoever from any damage, loss, injury, or death to me, my children, or my property which may arise out of or in connection with participation in any classes or activities conducted all or in part by Shall We Dance or West Virginia Ballroom, LLC. I further hereby voluntarily agree to waive my rights and that of my heirs and my assigns to hold West Virginia Ballroom, LLC, Shall We Dance, its owners, agents, staff, volunteers, assistants, employees, guest artists, faculty members, teachers, teacher assistants, instructors, coaches, spotters, and/or students as well as Ninja Zone liable for such damage, loss, injury, or death. I understand that I and my children should be aware of our physical limitations and agree not to exceed them. If I am signing this waiver for my child, I certify that I am the parent or legal guardian and have the authority to waive these rights.
Permission is also granted to West Virginia Ballroom, LLC, Shall We Dance, its owners, agents, and staff as well as Ninja Zone to take and use photographs of myself and my children for any publicity purposes whatsoever.
By continuing to register my child(ren) and/or clicking "I Agree," I acknowledge that I have read, understood, and further agree to be bound by all of the above statements.
