ENCORE DANCE ACADEMY LIABILITY WAIVER AND RELEASE FORM

ASSUMPTION OF RISK, WAIVER OF LIABILITY, AND INDEMNIFICATION AGREEMENT
As the parent or legal guardian of the above-named student (the "Participant"), I hereby acknowledge and agree to the following terms and conditions in consideration of the Participant’s enrollment in and participation in any classes, rehearsals, performances, events, or other activities (collectively, “Activities”) conducted or sponsored by Encore Dance Academy, located at 1000 West Locust Street, Canton, IL 61520 (the “Academy”).
Assumption of Risk
I understand that dancing and related physical activities carry inherent risks, including but not limited to falls, strains, sprains, broken bones, and other physical injuries. I voluntarily accept and assume all such risks on behalf of the Participant, whether foreseeable or unforeseeable, known or unknown, associated with participation in Academy Activities.
Medical Authorization
I certify that the Participant is physically fit to engage in dance and related physical activities. In the event of an emergency where I cannot be reached, I authorize Encore Dance Academy and its representatives to seek emergency medical treatment for the Participant. I understand that I am responsible for any medical expenses incurred.
Release and Waiver of Liability
To the fullest extent permitted by law, I hereby waive, release, and discharge Encore Dance Academy, its owners, officers, instructors, employees, volunteers, agents, and representatives (collectively, the “Released Parties”) from any and all claims, demands, causes of action, damages, or liabilities of any kind, whether in law or in equity, arising out of or relating to the Participant’s involvement in any Academy Activities, including but not limited to claims for personal injury, illness, property damage, or death, even if caused by the negligence of the Released Parties.
Indemnification
I agree to indemnify, defend, and hold harmless the Released Parties from any and all claims, liabilities, damages, costs, and expenses (including reasonable attorneys’ fees) arising out of or in connection with the Participant’s participation in Academy Activities.
Media Release (optional – remove if not needed)
I grant permission for the Participant to be photographed, videotaped, or otherwise recorded during Academy Activities, and for those images and recordings to be used for promotional purposes by Encore Dance Academy, including but not limited to social media, website, and print materials.
Severability
If any portion of this agreement is found to be invalid or unenforceable, the remaining provisions shall continue in full force and effect.
Governing Law
This agreement shall be governed by and construed in accordance with the laws of the State of Illinois.
