Assumption of Risk:

I, the minor's parent or legal guardian, understand the nature of the activities my child will be involved in at Chicago Contemporary Dance Theatre/Company, and the minor's experience and capabilities, and believe the minor to be qualified, in good health, and in proper physical condition to participate in such activities.
I have read the "Assumption of Risk" and agree to the terms.

Release of Liability:

I hereby release, discharge, covenant not to sue, and agree to indemnify and save and hold harmless each of the releasee's from all liability claims, demands, losses, or damages on the minor's account caused or alleged to be caused, in whole or in part by the negligence of the "releasees" or otherwise, including negligent rescue operations. I further agree that if, despite this release, I, the minor, or anyone on the minor's behalf makes a claim against any of the releasees named above, I will indemnify, save, and hold harmless each of the releasees from any litigation expenses, attorney fees, loss liability, damage, or any cost that may occur as the result of any such claim.

Medical Emergencies:

In any event of an accident or emergency I hereby authorize my child to be transported to a hospital for medical treatment and I hold Chicago Contemporary Dance Theatre/Company and it's representatives harmless in the execution of such. Additionally, I hereby agree to individually provide for all medical expenses which may be incurred by me or my child(ren) as a result of any injury sustained while participating at or for Chicago Contemporary Dance Theatre/Company.
I have read the "Medical Emergencies" and agree to the terms.

Refunds:
I understand that there is absolutely no refund for any payments made.

Enrollment Agreement & Acknowledgement

By enrolling my dancer with Chicago Contemporary Dance Theatre (Chi-CDT), I acknowledge that I have read, understand, and agree to abide by the policies, procedures, and expectations outlined in the Chi-CDT Student & Company Handbook, Parent Portal, and all future written communications issued by Chi-CDT.

I understand and agree that:

General Policies
My dancer is enrolling in the Chicago Contemporary Dance Theatre Company Pathway and will participate according to their assigned program (Pre-Company, Company II, or Company I).

Class placement, Company placement, choreography, casting, and performance opportunities are determined solely by the Chi-CDT coaching staff.

Placement decisions are based on ongoing evaluations and may change throughout the season.

Tuition & Financial Responsibility
Tuition is based on enrollment and reserves my dancer's place in class.
Tuition is not prorated for absences, vacations, holidays, illnesses, or other missed classes except as expressly approved by Chi-CDT.
I am responsible for all tuition, fees, and charges associated with my dancer's enrollment.
I authorize Chi-CDT to process scheduled tuition payments using my payment method on file in accordance with the selected payment plan.
I understand that late fees, declined payment fees, late pick-up fees, Company fees, and other applicable charges may be added to my account when appropriate.

Attendance & Participation
My dancer is expected to attend all required classes, rehearsals, performances, and Company activities associated with their placement.
Failure to maintain attendance or preparedness may affect choreography, casting, performances, or Company placement.

Health & Safety
I certify that my dancer is physically able to participate in dance activities unless otherwise disclosed to Chi-CDT.
I agree to notify the studio of any illness, injury, allergies, medical conditions, or restrictions that may affect my dancer's participation.
I understand that Chi-CDT staff does not administer medication except as required by law or in emergency situations.

Supervision & Student Safety
I understand that Chi-CDT supervision is limited to scheduled classes, rehearsals, performances, and approved studio activities within the

Chi-CDT studio suite.
I understand that dancers must notify the Front Desk before leaving the studio suite.
I understand that Chi-CDT does not supervise or assume responsibility for dancers who leave the studio suite without authorization or who enter other areas of the Bridgeport Art Center, including the first-floor café or common areas.

Parent Responsibilities
I agree to arrive on time for drop-off and promptly pick up my dancer following their scheduled activities.
I understand that late pick-up fees will be assessed according to Chi-CDT policy.
I agree to communicate respectfully with instructors, staff, volunteers, dancers, and other families.

Photography & Media
I understand that photographs and video may be taken during classes, rehearsals, performances, competitions, and studio events and may be used by Chi-CDT for educational, promotional, or marketing purposes unless I have submitted a written opt-out, if permitted by studio policy.

Handbook & Policy Updates
I understand that the Chi-CDT Student & Company Handbook may be revised as necessary during the season.
I agree to comply with all future written policies, procedures, schedules, and communications distributed by Chicago Contemporary Dance Theatre.

Acknowledgement

By selecting "I Agree" and completing my registration, I acknowledge that I have read, understand, and agree to comply with the policies and expectations of Chicago Contemporary Dance Theatre. I understand that failure to comply with these policies may result in disciplinary action, suspension, dismissal from the program, or other actions deemed appropriate by Chi-CDT.
