As a parent or guardian of a student at Danse d’Amour Studio, I acknowledge and agree to the following:

Age Confirmation: I confirm that I am over 18 years old.

Communication: I commit to staying on top of emails and promptly responding to communications from Danse d’amour Studio to ensure I am informed about my dancer's schedule, requirements, and studio updates.

Support: I am dedicated to supporting my dancer’s success by ensuring they attend classes regularly, participate in required activities, and have the necessary dance attire and equipment.

Engagement: I will actively engage with studio events, meetings, and parent-teacher conferences to stay involved in my dancer’s progress and development.

As the parent or legal guardian of a student at Mouvement Dance Studio, I acknowledge and agree to
adhere to all studio policies and procedures as set forth by the studio administration.
1. Withdrawals:
All student withdrawals must be submitted in writing to the studio administration. If I choose to
withdraw my child from the program without providing a minimum of 30 days’ written notice, I
acknowledge that I will be financially responsible for the remaining tuition for the entire dance
season, regardless of attendance.
2. Tuition & Fees:
I agree to make timely payments for all tuition, registration fees, costume fees, recital fees, and any
other associated program costs. I understand that late payments may result in late fees, and that my
child may be suspended from classes until the account is brought current.
3. Safety & Conduct:
I commit to ensuring that my child follows all safety rules, studio guidelines, and behavior
expectations as outlined by Mouvement Dance Studio. This includes, but is not limited to:
– Showing respect to instructors, staff, and fellow students
– Following the designated dress code for all classes and events
– Conducting themselves safely and appropriately during classes and while on studio premises
– Attending classes regularly and arriving on time
4. Disciplinary Actions:
I understand that the studio reserves the right to take appropriate disciplinary actions should these
rules and expectations not be followed. This may include verbal warnings, parent conferences, or
dismissal from the program in cases of repeated or serious violations.
5. Media Release (Optional):
Unless I notify the studio in writing otherwise, I give permission for photos and videos of my child taken
during classes or events to be used for studio promotional materials, including social media and
marketing purposes.
6. Assumption of Risk & Liability Waiver:
I understand that dance is a physical activity and, like all physical activities, carries some risk of injury. I
hereby release and hold harmless Mouvement Dance Studio, its instructors, staff, and affiliates from
any liability or claims arising out of any injury or accident occurring on studio premises or during
studio-sanctioned activities.
By signing below, I affirm that I have read and understood the policies outlined above. I agree to
comply with these terms and acknowledge that failure to do so may result in financial liability and/or
dismissal from the program
