BETHANY ANN HELLEN STUDIOS, INC.
RELEASE OF LIABILITY, WAIVER, AND STUDIO POLICIES AGREEMENT
1. ACKNOWLEDGMENT OF POLICIES AND DRESS CODE
By checking the agreement box below, I acknowledge that I have read, understand, and agree to abide by all the official policies, tuition guidelines, and dress codes established by Bethany Ann Hellen Studios, Inc. (the "Studio"), which are posted on the Studio website and registration materials.
2. ASSUMPTION OF RISK
I understand that participation in dance instruction, rehearsals, performances, and related physical activities (collectively, the "Activities") involves inherent risks of physical injury, illness, or property damage. I acknowledge that while proper body alignment, technique, and safety are consistently taught and reinforced by the Studio, some circumstances are entirely beyond the control of the instructors and the Studio staff. I knowingly and voluntarily assume all risks, both known and unknown, associated with my participation (or my child’s participation) in these Activities.
3. WAIVER AND RELEASE OF LIABILITY
In consideration of being allowed to participate in the Activities, I, on behalf of myself, my minor child (if applicable), and our respective heirs, executors, administrators, and assigns, hereby release, waive, forever discharge, and hold harmless Bethany Ann Hellen Studios, Inc., its owners, directors, officers, employees, independent contractors, and instructors from any and all claims, demands, rights, causes of action, or liability for personal injury, illness, medical expenses, or property damage/loss sustained while on the Studio premises, in transit to or from Studio-sanctioned events, or while participating in any Studio Activities, whether arising from the negligence of the Studio or otherwise.
4. MEDICAL DISCLOSURES, EMERGENCIES, AND INSURANCE
I certify that the participant is in good physical condition and fully capable of participating in the Activities. I agree that I have fully informed the Studio in writing of any and all past or current injuries, medical conditions, allergies, or physical limitations that could affect safety or the ability to dance.
In the event of a medical emergency, I hereby grant permission to the Studio staff and representatives to authorize emergency medical treatment, including first aid, transportation to a medical facility, and necessary physician services, if I (or the minor participant's parent/legal guardian) cannot be reached immediately. I agree that I am solely responsible for all medical and transportation expenses incurred, and that my personal health insurance will serve as the primary coverage.
5. PERSONAL PROPERTY
I understand and agree that Bethany Ann Hellen Studios, Inc. is not responsible or liable for any personal property that is lost, damaged, or stolen inside the studio, in the dressing areas, common spaces, or in the surrounding parking areas.
6. PHOTOGRAPHY AND MEDIA RELEASE
I hereby grant Bethany Ann Hellen Studios, Inc. the right and permission to take, use, publish, and reproduce photographs, video recordings, and audio recordings of the participant taken during classes, rehearsals, performances, or events for promotional, marketing, educational, or website purposes.
Note: If you wish to opt out of public photos, you can update your preferences at any time in the Parent Portal by navigating to "More" > "Waiver & Release" > "Photo Opt-Out."
7. SEVERABILITY
I expressly agree that this waiver and release is intended to be as broad and inclusive as permitted by law. If any provision or portion of this agreement is held to be invalid or unenforceable by a court of law, the remaining provisions shall continue in full legal force and effect.

I have read this document in its entirety, fully understand its terms, and understand that I am giving up substantial legal rights by signing it voluntarily.
