I hereby register as a student at Studio V Performing Arts Center. I agree to pay tuition in full by the 5th of each month and no later than the 10th, after which a $25 late fee will be applied.

I understand that:

There are no credits or refunds for lessons missed by the student. However, make-up classes may be scheduled through the front desk.

To drop a class, a parent/guardian must call or visit the studio before the 1st of the month to provide notice. Tuition will continue to be charged monthly until notice is received.

Registration fees and tuition payments are non-refundable.

I authorize Studio V to automatically process my tuition and fees using the payment method I have selected. I also understand that all payments made by credit card, debit card, or any non-ACH method will incur a 4.5% convenience fee, which will be added to my total balance. I acknowledge and accept this fee as part of my payment authorization.

I hereby release Studio V Performing Arts Center, Utah Dance Works LLC, and staff from all claims of damages or injury suffered by the above registered student in connection with or by their association with the studio, excepting only gross negligence, relative to actions of decisions that concern or affect my child. This includes heirs who may not act in my behalf. Studio V is not responsible for children not picked up 10 minutes after class, unless previous arrangements are made.

I authorize Studio V Performing Arts Center to use photography/video of the individual dancer in promotional material.
