Payment Policy for ALL Inspired Dance classes:

1. Tuition is Due on the 5th of each month. Payment that occurs after after the 15th will incur a $25 late fee. To avoid $25 late fee, please make payment before the 15th. Questions about payment must be emailed to the director.

2. Registering for a class, means that as a parent or guardian, you are agreeing to the financial obligations of the class, including payment, attendance, and session time frame. In the case of unexpected events or emergencies, a Parent MUST communicate dancer's intent to withdraw. The Parent or guardian is still responsible for the session/season payment. WITHDRAWING EARLY during the semester/season will result in a CANCELLATION FEE OR OBLIGATION TO PAY THE REMAINING SESSION FEE.
The fee will be at least a minimum of next months payment upto the remaining months of tution. Upon communication, Inspired dance will determine the situation and decide if all or a part of the remaining balance is owed. Cnce the fee is posted, the cancellation fee must be paid for this request to be valid/final.
Please understand that commitment to the session or the season is important.

3. Reminder: Once you register for a class, Monthly and registration fees apply. Payment is non-refundable. Dropping out does not not release you of commitment to payment for that session/season. Make sure you read the Withdrawal form 7/or policies on our website.

Waiver Form for Medical, Liability, and Photo

Please read the following. In the event there is an emergency, we will call legal guardians first. If we can't get a hold of a parent or guardian then further action will be taken.

Medical Consent: In the event of injury, Inspired Dance will call the parent/guardian first. If the guardian is unreachable, I hereby authorize the program officials of Inspired Dance and all other cooperating agencies, employees, officials, or managers thereof to arrange for medical services as may be deemed reasonable and necessary to the welfare of the injured, and I do hereby release Inspired Dance and their employees and all others involved with the activity from all liability in taking such action, including all action which may be contrary to personal and/or religious beliefs. I, the undersigned, have read this Release and Consent to medical treatment & understand all its terms. I execute it voluntarily and with full knowledge of its significance.

Liability Release: I do hereby agree to release Inspired Dance and West Ada Schools, Shine Fitness, Evergreen Health Clinics, and all other cooperating agencies, employees, officials, or managers thereof from all liability for damages by reason of injuries or property damages that may be sustained as a result of participation in this program.

Photo/Video Release: I, the undersigned, hereby give Inspired Dance, its agents, and/or assignee permission to use the photographs, motion pictures or any reproductions of the physical likeness to use for website, social media or flyers to help promote inspired dance and show what makes us unique. Use of any material will be done in an appropriate and modest manner.
I relinquish all rights, title, and interest I may have in the finished pictures. I waive the right of prior approval and hereby release Inspired Dance, its agents, and/or assignee from any and all claims from damages of any and all kinds based on the use of said material. I hereby warrant that I am a parent or legal guardian of the subject of photography, who is under eighteen years of age, and am competent to act in his/her behalf insofar as the above is concerned.
