Rage Academy of Dance, LLC – Accident & Liability Waiver

By enrolling in programs at Rage Academy of Dance, LLC, I acknowledge and agree to the following:

I understand and voluntarily assume all risks related to my own or my child’s participation in classes, camps, private lessons, intensives, choreography, or any other activity hosted by Rage Academy of Dance, LLC. These risks may include injury, illness (including exposure to communicable diseases), accidents, property damage, natural disasters, and other unforeseen circumstances — whether arising from negligence or otherwise.

I certify that I and/or my child are physically fit to participate and have not been advised otherwise by a medical professional. I agree that Rage Academy of Dance, LLC is not responsible for any injury or illness that may occur before, during, or after participation.

I hereby waive, release, and discharge Rage Academy of Dance, LLC, its owners, officers, employees, volunteers, and representatives from any and all liability, including but not limited to negligence, for injury, illness, loss, or damages of any kind. I also agree to indemnify and hold harmless all parties from any claims resulting from participation.

This waiver applies to all activities associated with Rage Academy of Dance, including travel to and from events. I understand this is a legally binding agreement.

PARENT/GUARDIAN CONSENT (If participant is under 18):
I am the parent or legal guardian of the participant. I consent to their participation and agree to all terms stated above on behalf of myself and my child.

✅ Rage Academy of Dance, LLC – Studio Policies & Agreements
By enrolling at Rage Academy of Dance, LLC, I acknowledge and agree to the following:

🎭 Behavior Policy
Our goal is to provide a fun, positive, and educational environment in every class. To help us achieve this, students must maintain appropriate behavior.
Disruptive or disrespectful behavior toward instructors or classmates will not be tolerated. If a behavior issue arises, the instructor will contact the parent to discuss solutions. Sometimes behavior concerns relate to class placement, and open communication with instructors is encouraged.
Please do not interrupt a class in progress. If you need to speak with an instructor, do so before or after class, respecting their scheduled transitions.

🧼 Hygiene Policy
In a physically active environment like dance, hygiene is important for everyone’s comfort and well-being.
Please ensure that:

Dancewear is cleaned after each use.

Hair is clean and pulled back properly.

Feet are washed before class.

Deodorant is used when necessary (spray deodorant is available at the studio).

Strong perfumes or scents are not worn to class, as they may trigger allergies or migraines in others.

🤒 Illness & COVID-19 Policy
For the health of all dancers and staff:

Do not attend class if you are sick or have shown symptoms (fever, vomiting, etc.).

Students must be fever-free for 24 hours without medication before returning.

If your child stayed home from school due to illness, they should not attend dance that day—even if symptoms subside later.

Please do not arrive more than 10 minutes early for class.

Only dancers and approved staff should enter the studio—spectators are discouraged unless invited.

📸 Media Consent
By enrolling, I grant permission for my child to be photographed or recorded during classes and events. These may be used for promotional or educational purposes (including but not limited to: showcase footage, website, social media, flyers, ads, etc.).
If you do not want your child’s image used, please ensure the appropriate box is unchecked in your DSP Parent Portal profile.

💳 Tuition & Payment Terms
Tuition is due on or before the 10th of each month.

If payment is not made by the 10th, the card on file will be charged automatically via autopay.

A $15 late fee will be applied to any account not paid or declined after the 10th.

All fees—including tuition, costume payments, competition fees, and other charges—are non-refundable once processed, unless a refund is approved by management under special circumstances.

If a dancer needs to withdraw from class, written notice must be submitted before the next billing cycle to avoid further charges.

Missed classes will not be refunded or prorated, except in the case of an extended absence due to medical reasons, with official documentation provided.

🏫 Studio Rules
Students may only be in studio rooms when actively enrolled in that class.

Dancers must be dressed and ready before class begins.

No street shoes in studio rooms—please change into dance shoes before entering.

Young children should not be left unattended in the lobby.

No gum, food, or snacks in the dance rooms.

Show respect to all staff and fellow dancers.

Bring a closed water bottle to every class.

Most importantly—Have Fun!

By checking the agreement box during registration, I confirm that I have read and agree to all of the above policies, and that I will review them with my dancer(s) as needed.
