You must be over 18 to register for an account. Fees paid are non-refundable, (credits may apply). Please be sure to review and sign the online waiver prior to attending any class.

Tuition Policies:
Tuition is due the 1st of each month. If tuition is not paid by the 10th of the month, a $10 late fee will be charged. Late fees also apply to recital fee ($5 after due date), costume fees ($20 after due date), and competition fees ($10 per routine after due date). *There is a 2.75% convenience fee on credit card payments. *There are no refunds on any fees paid, credits only (no credit or refund for competition fees paid, or costumes that have been ordered).

Studio Etiquette:
Please drop off student at most 15 minutes prior to class, and pick up no later than 15 minutes post class (if extenuating circumstances occur, please contact owner). Please no smoking in the studio, nor directly outside the doors.

Safety Policies:
Dancers must wear a coverup in and out of studio. Dancers need to wait for parents inside of the facility.
Dancers who drive themselves are asked to walk in pairs or groups, to and from cars. Dancers are expected to follow safety rules given by instructors during classes, regarding execution of skills, as well as body placement in the facility.

Waiver:
I fully understand that Stars Unlimited staff members are not physicians or medical practitioners of any
kind. With that fact in mind, I hereby release Stars Unlimited staff to render temporary first aid to my
child or children in the event of any injuries or illness, and if deemed necessary by the staff to seek medical help, including calling of an ambulance to a hospital for said child shall it be deemed necessary.

We, the staff of Stars Unlimited recognize our obligation to make our students and their parents aware of
the risks and hazards associated with the sport of dance. Students may suffer injuries, possibly minor, or
serious in nature. Parents should make their children aware of the possibility of injury and encourage their children to follow safety rules and the instructors instructions. Stars Unlimited, its instructors and staff members, will not accept responsibility for injuries sustained by any student during the course of dance instruction, acting classes, voice lessons, open workouts, or in the course of any exhibition, competition, or clinic in which he or she may participate or while traveling to or from the event. With the above in mind, and being fully aware of the risks and possibility of injuries, I consent to have my child or children participate in programs offered by Stars Unlimited. I, my executors or other representatives, waive and release all rights and claims for damages that my child or I may have against Stars Unlimited and its representatives.

The novel coronavirus, Covid-19, has been declared a worldwide pandemic by the World Health Organization. Covid-19 is contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations prohibited the congregation of groups of people.

Stars Unlimited has put in place preventative measures to reduce the spread of Covid-19; however cannot guarantee that you or your child(ren) will not be infected with Covid-19.

By signing this agreement, I acknowledge the contagious nature of Covid-19 and voluntarily assume the risk that my child(ren) and I may be exposed to.

I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) or myself, illness, damage, loss, claim, liability, or expense, of any kind that I or my child(ren) may experience or incur in connection with my child(ren)'s attendance at Stars Unlimited, or participation in studio programming. On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless Stars Unlimited, its staff, agents, and representatives, of and from the claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any claims , whether a Covid-19 infection occurs before, during, or after participation in any studio program.
