As the legal parent or guardian, I release and hold harmless From the Top Studio of Dance its owners and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of From the Top Studio of Dance, its owners and operators or in route to or from any of said premises.
The undersigned gives permission to From the Top Studio of Dance, its owners and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. I hereby declare any physical/mental problems, restrictions, or condition and/or declare the participant to be in good physical and mental health.
Tuition is due when indicated. If accounts are over 10-days late, there will be a $15.00 late fee applied to the account balance. There is a $35.00 returned check charge for any checks returned by the bank.

Assumption of Risk
I understand that participation in dance classes, rehearsals, performances, competitions, workshops, camps, and related activities at From the Top Studio of Dance involves physical activity and carries inherent risks, including but not limited to muscle strains, sprains, broken bones, falls, collisions, and other injuries.

I voluntarily allow my child to participate in these activities and acknowledge that I assume all risks associated with participation, whether known or unknown.

Health Certification
I certify that my child is physically able to participate in dance activities and has no medical condition that would make participation unsafe, except as disclosed via email to Info@FromTheTopDance.com

I agree to notify From the Top Studio of Dance of any changes to my child's health that could affect participation.

Release of Liability
To the fullest extent permitted by law, I, on behalf of myself, my child, and our heirs, release, waive, and discharge From the Top Studio of Dance, its owners, employees, instructors, assistants, volunteers, contractors, and agents from any and all claims, demands, causes of action, damages, losses, or liabilities arising out of or relating to my child's participation in studio activities, except to the extent caused by gross negligence, reckless conduct, or intentional misconduct.

Medical Treatment Authorization
In the event of an emergency and if I cannot be reached, I authorize From the Top Studio of Dance to obtain emergency medical treatment for my child as deemed necessary. I understand that I am responsible for any medical expenses incurred.

I understand that I am responsible for reviewing and following all studio policies regarding attendance, tuition, dress code, behavior, performances, and safety. I agree to comply with all policies established by From the Top Studio of Dance.

I have carefully read this Assumption of Risk, Release of Liability, and Medical Authorization. I understand its contents and sign it voluntarily. I acknowledge that by signing this document, I am giving up certain legal rights on behalf of myself and my child to the extent permitted by law.
