I agree to abide by the Dance Addiction Constitution and Parent/Student Code of Conduct. I understand all that is included in these contracts and agree to the terms thereof. I understand that failure to abide by these contracts may result in the removal of myself and my child from Dance Addiction Dance Studios. I agree to represent Dance Addiction to the best of my ability.
I do hereby fully release and discharge Dance Addiction, its officers, agents, servants, and employees from any and all claims from injuries, damage or loss which I (or dependents under 18 years of age) may have or which may occur to me (or dependents) on account of my (their) participation in the above Dance Addiction Studio sponsored activities. I further agree to hold harmless and defend Dance Addiction Studio, its officers, agents, servants, and employees from any and all claims resulting from injuries, damages, and losses sustained by me (or dependents under 18 years of age) and arising out of, connected with, or in any way associated with these activities sponsored by Dance Addiction.
