My dancer and I will read the Welcome Letter and adhere to the rules.

Authorization:
By signing below, I authorize Charla's School of Dance to automatically withdraw payments from the account/card I have entered for recurring charges on my account. I understand:

Payments will be withdrawn on or after the due date each billing cycle.
It is my responsibility to ensure sufficient funds are available.
I may cancel this authorization at any time by notifying Charla's School of Dance in writing at least 2 days before the next scheduled payment.
Returned or declined payments may result in fees or service interruption.
