I understand a deposit of $30.00+GST is payable upon initial enrolment which covers a trial period of 3 classes.
I understand that the balance of the remaining fees plus $10.00 registration fee (new students only) is payable on the 4th week.
The deposit is deducted from the term’s fees.
I understand I am committed for the entire term’s fees regardless of any classes missed due to sickness or absence. When ceasing the enrollment, an email must be forwarded to the office at least 10 days prior to the commencement of the next term. I understand failure to do so means the student is enrolled for the next term and the appropriate fees may be payable.
I give permission for my child’s photo to be used in promotional advertising including social media for the dance school.
I understand that whilst all care is taken no responsibility will be held by Heather Evans Dance Studios for any injuries.
