Assumption of Risk: I make the following acknowledgments on behalf of myself and / or all minors, who I hereby confirm that I am the parent or legal guardian of (the "Student(s)"), who will be participating in the dance classes, camps, and related activities provided by Grace Dance Creation Studio Inc. (the "Activities") and any references below to "I" or "my" shall be on behalf of and include both myself and the Student(s). I understand that participation in the Activities may involve risks, dangers and hazards, including the risk of serious injury, any illnesses such as COVID-19, death, or property damage. I acknowledge that I am voluntarily participating in the Activities. I freely accept and fully assume all of the risks, dangers and hazards involved and the possibility of injury, death or property damage, or any illnesses whether caused by the negligence of Grace Dance Creation Studio Inc. or otherwise.
In the event of an emergency, if the emergency contacts that I have listed for my child cannot be reached, I hereby grant Dance Creation Studio the authority to arrange for transport of my child to the nearest medical care facility.
Payment Plans All Payment Plans — including Trainee Program Payment Plans, CBC and Company Payment Plans — are set up with a credit card and require an active card in good standing to remain on file in the Parent Portal. Payments are automatically withdrawn on the 1st of each month, unless a special arrangement has been made with the school. Payment Plans cannot be set up using debit, cash, or EMT. Families wishing to make a payment more than one week in advance of the scheduled withdrawal must notify the office immediately after payment is made to prevent a double charge. Without notice, the scheduled withdrawal will still occur. ⸻ Term Refund Policy • Before the term begins: Refunds can be issued in full, minus a $15 processing fee. • After the first class but within the first week: Refunds can be issued minus a $15 processing fee and a single drop-in class fee. • After the first week: No refunds will be issued. ⸻ Payment Plan Refund Policy A minimum of 30 days’ written notice is required to cancel a monthly payment plan. Payments will continue during the notice period, and the plan will end with the next scheduled payment following the 30 days. ⸻ Drop-In Class Refund Policy Drop-in fees are non-refundable for cancellations made less than 24 hours before class. Cancellations made more than 24 hours in advance will receive a credit that can be used at any point during the term. ⸻ Camp Tuition Refund Policy No refunds are available for camp tuition.
Dance Creation has my permission for my child/children's photographs/videos to be used for publicity and advertising in the studio, on the Dance Creation Studio website, Facebook page, Instagram, and in the community unless I opt out by submitting a written request or choosing Photo Opt Out in the Parent Portal.
Release & Waiver of Liability: I hereby expressly waive and release any and all claims which I have or may in the future have against Grace Dance Creation Studio Inc., its affiliates, directors, officers, employees, agents, representatives, shareholders, successors and assigns (the "Releasees") arising out of or attributable to my participation in the Activities, due to any cause whatsoever, including without limitation the negligence of the Grace Dance Creation Studio Inc. or any of the other Releasees, breach of contract, or breach of any statutory or other duty of care owing under any legislation or at common law or otherwise. I covenant not to make or bring any such claim against Grace Dance Creation Studio Inc. or any of the other Releasees, and forever discharge and release Grace Dance Creation Studio Inc. and all other Releasees from liability under such claims.
