I acknowledge that my child’s participation in dance classes with Megan Meyers (Princess Ballerinas, Dance Adventures, and/or Dance Fusion) involves physical activity, and I understand the risks associated with movement and dance. I give permission for my child to participate in the trial class and understand that I am responsible for supervising my child before and after class. I agree to the above terms and give permission for my child to participate.
