I acknowledge that my child’s participation in the Princess Ballerinas trial class involves physical activity, and I understand the risks associated with movement and dance. I agree that Princess Ballerinas and its instructors are not responsible for any injury that may occur. I give permission for my child to participate in the trial class and understand that I am responsible for supervising my child before and after class.
