You must be 18 years old or older to register for an account.

In exchange for my child's voluntary participation with Simply Dance, I agree to the following:

1) I recognize that there are certain inherent rights and risk of injury associated with the activity of dance. I assume full responsibility for any personal injury to my child. I further release and discharge Simply Dance, its employees, instructors, and all affiliated entities for any injury, loss or damages arising out of participation in class and all other activities associated with Simply Dance.

2) I have read and understand all "Studio Info" and will uphold all the information given.

3) I have read and understand this agreement and I understand that I am voluntarily releasing certain legal rights.

I agree and understand that if my account is 45 days past due, my child/children will no longer be able to participate in class.
