DANCE TUITION AND PARTICIPATION WAIVER
I, the parent/guardian/carer of Minor, hereby acknowledge and consent to the following:
I recognise that dance, acrobatic and fitness programs, or any other class or tuition offered by Base181 Studios (herein after Base181require physical exertion that is strenuous in nature and may have the potential to cause physical injury. I am fully aware of the risks and hazards involved and choose to still allow the abovenamed child to participate, despite of this knowledge. I agree to assume full responsibility for all tasks, injuries or damages, known or unknown, which may be incurred as a result of participation in these activities and further, will hold harmless Base181 for any damages and injury that may be sustained as a result of failure, in whole or part, of the participant to follow instructions given;
2. I hereby voluntarily, fully and forever waive, release and discharge Base181 Studios (herein after Base181), its owners, officers, directors, agents, employees, volunteers, and all other persons or entities acting in any capacity on its behalf from any and all losses, liabilities, claims, expenses, demands, actions, damages, injuries, causes of action, and rights of action which are related to, arise out of, or are in any way connected with my childs participation in dance or acrobatic tuition at, with, or sponsored or administered by Base181. The foregoing waiver of liability includes, without limitation, damages or injury resulting from the negligence of Base181 Studio (whether such claims are based upon breach of contract, breach of warranty, or any other legal theory) or from any other cause or causes. I hereby agree not to institute any legal proceedings against Base181 for damages, whether to property or person, that my child may have sustained during participation in the dance, acrobatic or other tuition. I further hold Base181 harmless from any and all loss, liability, claims or expenses suffered which are in any way connected with my childs (or my own) participation in activities of Base181;
3. I hereby acknowledge and understand that it is my sole responsibility to consult with a physician prior to the abovenamed child beginning any classes offered by Base181. I confirm that such child is physically fit and has no medical condition that would otherwise prevent full participation. I understand that any advice of any directors, agents, employees, volunteers or other of Base181, is not to supersede the advice of a physician;
4. I hereby acknowledge and agree to mention any changes in the health of the abovenamed child to the instructor/teacher or associated employee of Base181 prior to the commencement of any class. This includes but is not limited to sprains, common colds, flu, pregnancy, general aches and pains and more;
5. The above acknowledgement is not only for participation in any dance, acrobatic or other tuition within the studio, however, extends to any external events in which I have agreed to participate in (on behalf of the abovenamed child), inclusive of, but not limited to, external examination venues, fetes, dance concert venues, eisteddfod venues and any other place which is considered external to the Base181 Studio.
6. If this form is provided to me but remains unsigned by me, by allowing the child in my care to participate, I am hereby accepting the above by my actions;
7. Base181 Studios takes all precautions and follows the guidelines of the New South Wales Goverment in relation to the current COVID climate. This includes ensuring all equipment is sanitised between each use and that upon arrival and exit all participants have their hands sanitised. By signing this waiver you agree that this studio releases itself from any liability surrounding their policies and procedures and anything in slight connection to the COVID 19 climate.
8. Release for photography and videography: by checking the box below, I give permission for Base181 Studios to use video of photographic footage in which my child appears (and which is taken during the time at the Studio or during any external activity) for the purpose of (however, is not limited to) displaying publicly, distributing, publishing for online or offline advertisement, social media, brochures, newsletters or any other purpose in which Base181 sees fit.
General Terms
Enrolment is presumed for the entirety of 2025, I understand that fees will continue to be charged until BASE181 STUDIOS is notified in writing of withdrawal from classes.
To withdraw from a class, 30 DAYS notice from scheduled class day must be provided in writing to DANCE@BASE181STUDIOS.COM. Normal class fees will be charged if notice given is less than 30 DAYS.
If you choose to cancel, the cancellation process requires written notice, and a 30-day calculation period will apply. If your membership falls within this 30-day window, the corresponding fee is payable. If not, no further debits will be taken after the conclusion of the 30-day period.
I understand that any payments made via CREDIT CARD AND/OR BAND ACCOUNT as the payment method incur a transaction FEE charged by the payment provider.
AN ANNUAL REGISTRATION FEE IS CHARGED AT THE COMMENCEMENT OF THE DANCERS ENROLMENT. THIS IS A NON REFUNDABLE FEE AND WILL BE ADDED TO YOUR FIRST MEMBERSHIP INSTALMENT or at the time of enrolment.
Students will not be able to participate in classes if payment details are not on file or payment in advance is not received by theIR FIRST CLASS.
Students will not be able to participate in any BASE181 STUDIOS events if account is not up to date and there are any outstanding amounts owed on account.
BASE181 STUDIOS reserves the right to pass any outstanding fees onto a Debt Collection Agency, this will incur a 20% surcharge on the balance of the account.
Your monthly membership will activate the day that you enrol (after paying your ANNUAL REGISTRATION fee). Membership fees will be automatically paid on the day you enroll every month after. BASE181 STUDIOS offerS 40 weeks of high-quality dance classes over a 12 month period;. I UNDERSTAND I AM NOT BEING CHARGED FOR HOLIDAYS & THE MEMBERSHIP PROGRAM already accounts for holidays.
Direct Debit Terms
As a DIRECT DEBIT customer with Base181 Studios , through our payment System, I agree that:
I am responsible for entering and updating when necessary my bank account and/or credit card details via THE DIRECT DEBIT LINK FOUND ON THE STUDIO APP .
BASE181 STUDIOS, through their payment provider has the authority to debit my account or credit card the full balance owing on my AGREED MONTHLY MEMBERSHIP.
The registration fee varies based on the time of enrolment during the year. Specific details will be shared at the time of registration, which typically occurs on the 1st of each month unless otherwise specified.
I will pay the associated transaction fees which are detailed and emailed at the time of filling out the direct debit form.
A failed payment fee of $4.40 will be added to my Base181 Studios account if my direct debit payment fails.
Where a direct debit payment fails in 3 consecutive times, students will not be able to participate in classes and any Base181 Studios events until all outstanding amounts are paid in full.
Redeeming your Active and Creative kids Vouchers
If you wish to redeem your Active or Creative kids voucher you must upload your voucher information to our studio app under the tab "Redeem Vouchers" by the 20th of each month. If you do so after this period, your voucher will be redeemed the following month. Please note, if you're newly enrolled your voucher will be used for your second instalment with Base181.
Annual Registration
This registration fee is a one off annual payment as class sizes are restricted, this payment will secure your child's/children's place in their classes. This fee is nonrefundable and is added to your first instalment.
Communication Policy
At Base181 Studios, clear and consistent communication is a priority to ensure all families stay informed.
Primary Source of Communication
Our main source of communication is our Studio App. All important updates, announcements, schedules, and other essential information will be shared through the app. You can access and join the app by visiting: https://www.base181studios.com/bandlinks.
Additional Communication Methods
As a secondary measure, we may send reminders via email and text message, but the Studio App remains the primary source for all detailed information.
Parent Responsibility
It is the responsibility of parents/guardians to regularly check the Studio App for updates and notifications.
Please ensure your contact information in the Studio App is accurate and up to date to avoid missing important announcements.
Pick-Up & Drop-Off Policy
Students must be collected immediately after class. Should you be running late, please call or text 0456 561 794 before the end of the class.
Students are prohibited from leaving the studio unaccompanied at any time.
Parents dropping off and picking up children must use Base181 allocated parking only. Please respect our neighbours.
Teacher supervision extends only to students in class. Parents/guardians are responsible for supervising their child before the first class and after the last class.
Base181 Studios encourages parents to find appropriate parking and pick up their child from inside the studio. Parents who collect their child from the car park or street do so at their own risk.
Studio Rules
Parents, students, siblings, and friends are not permitted inside the dance studio unless accompanied by Base181 staff. Unless invited by a teacher to watch a class, parents and friends are not permitted.
A large television with CCTV footage is available for parents to view their child’s class.
Recording or photography from mobile/multimedia devices is strictly prohibited.
Students and their parents are liable for any damages caused to studio property.
By entering the premises, parents and/or guardians accept that risks may occur in attending or participating in events or activities at the venue. Base181 Studios does not accept responsibility for injuries due to attendance, participation, or the actions or negligence of others.
Parents are advised that some physical contact may be necessary for teachers to demonstrate exercises, techniques, or administer first aid.
Students may be photographed or filmed by Base181 Studios for social media, promotional materials, and productions. Parental permission is assumed unless otherwise advised.
Uniform/Dress Code Policy
Uniform: Students must wear the designated Base181 Studios uniform for their class style.
Footwear: Appropriate footwear is required for each dance style (e.g., jazz shoes for Jazz, sneakers for Hip Hop).
Hair: Hair must be neatly tied back and secured away from the face to allow full focus and avoid distractions.
Jewellery and Accessories: For safety reasons, jewellery (e.g., necklaces, bracelets, dangling earrings) should not be worn during class. Minimal, non-distracting accessories (e.g., small stud earrings) are allowed.
Health Policy
Parents are required to provide all health, behavioural, and medication information during enrolment. This information helps us prepare for your child’s care while at the studio.
Any child diagnosed with Asthma or Anaphylaxis MUST provide an individual Health Management Plan completed by a medical practitioner before their first class.
All medication must be signed in and out by a parent or guardian before and after class. Medication must be in its original packaging and clearly labelled with the student’s name, correct dosage, and frequency.
In the event of a medical emergency, and if a parent cannot be contacted, Base181 Studios is authorised to seek medical advice for the child.
Children with learning or behavioural difficulties must provide Base181 Studios with information about their needs to ensure we can deliver the best program for all participants.
Base181 Studios holds no responsibility for any conditions, known or unknown. Participation is at the guardian's risk.
Make-Up Class Policy
Make-up classes must be scheduled within the same term as the missed class.
Make-up classes are subject to availability and must be booked in advance.
Walk-ins for make-up classes are not permitted.
Unused make-up classes cannot be carried over and will expire at the end of the term.
