I understand this registration contract represents a financial commitment for the classes I have indicated. Refunds are only given before the first week of classes. Four make-up classes are allowed per session and must be completed during the same session. I certify that this student is in good health and capable of participating in all classes. I agree that I cannot hold SMAPA or any agent, representative, faculty member, or employee liable for injuries or illness contracted by the student while participating in SMAPA‘s activities, online or in the studio. I also agree that I cannot seek indemnification, reimbursement, or any other type of compensation from SMAPA, a faculty member or employee for any injuries or illness contracted while a student/participant of SMAPA. I understand that SMAPA cannot be responsible for any lost or stolen items.

In the event of a medical emergency, SMAPA has my permission to authorize first aid and /or lifesaving treatment to the participant listed above. In the event of an emergency, every effort will be made to contact me immediately. If I cannot be reached and emergency care is necessary, SMAPA will not be responsible for any medical charges incurred.
