Risk:
By accepting this waiver upon registration in any class at Living Skies School of Irish Dance (LSSID), I understand and acknowledge that participation in dance/movement/art classes may result in personal injury including, but not limited to injury to bones, joints, ligaments, muscles, tendons, internal organs, and other aspects of the skeletal system and potential impairment to other aspects of the body, and in rare occurrences, death, complete or partial paralysis or brain damage and property damage or loss. The definition of participating in a dance program includes but is not limited to: it is an activity that uses exercises and dance routines made up of elements from dance, stretching, gymnastics and yoga as a tool to strengthen the body, improve alignment, flexibility, body awareness, performance quality, expression and more for performances and, occasionally competition. I fully understand these risks and hereby agree to participate in a dance program voluntarily and at my own risk.
Rules / Conduct:
I understand that the rules and regulations are designed for the safety and protection of participants and hereby agree to abide by the rules and regulations set down the by studio. I have read the Dancer Code of Conduct and agree to conduct myself in a responsible manner as outlined in that document.
Liability:
In consideration of your acceptance of my entry in any class, rehearsal or performance, I, intending to be legally bound, agree to RELEASE, SAVE HARMLESS AND INDEMNIFY the teachers, the business, the staff, the organizers and/or its agents from and against all claims, actions, costs and expenses and demands in respect to death, injury, loss or damage to my person or property whatsoever and howsoever caused, arising out of, or in connection with my association with or entry in the above sanctioned activity or which may arise out of my travelling to or participating in and returning from classes or performances, competitions, festivals, or other events related to LSSID.
I further agree to HOLD HARMLESS LSSID, the teachers and staff members, the organizers and/or its agents from any and all actions, claims, demands, losses, judgments or costs of any nature to any third party resulting from my association or entry in the said sanctioned class and I agree not to make any claims or take any proceedings against any person, society, corporation or other entity who might contribution or indemnity from LSSID, the teachers and staff, the organizers and/or its agents in respect of matters which are subject of this Release. I agree that this Release shall bind my heirs, executors, administrators and assigns.
I confirm that I am of full age of 18 years, if student is under 18 years of age, parent/guardian must sign below, and have read, understood and agree to the contents of this informed Consent Form in its entirety. I confirm that I have read a copy of the studio rules and regulations and will abide by them.
I confirm that I have read and understand the refund policy of the school.

MEDIA RELEASE
I hereby grant LSSID the right to use, without payment of any fee or charge, any written information, photograph, video tape or other visual media of myself or my child taken during any class, rehearsal, or performance event for the purpose of furthering the LSSID objectives.

CANCELLATION / MISSED CLASS POLICY
- Lessons that are cancelled by the teacher will be made up at a later date in the year.
- Lessons missed by the student will not be made up under any circumstances.
- Dance fees are non-refundable for each trimester once that trimester has begun.
- Dance lessons cancelled before the start of a trimester will be refunded or post-dated cheques returned at the written request of the dancer/parent.

DANCER/FAMILY CONDUCT
Students and parents are expected to conduct themselves in an appropriate manner while at the LSSID studio and at LSSID performances and events. Politeness, courtesy and honesty are all highly valued. Proper respect for the instructors and fellow students is expected of all students, parents and siblings.
Every student and parent/guardian should behave in a respectful, responsible manner both on site and at performances. LSSID reserves the right to suspend or dismiss any student or parent/guardian whose attitude, attendance or conduct is found to be unacceptable. Any student or parent/guardian who disrupts class or approaches a teacher, staff member, another student or parent/guardian with the intent to disrupt, cause dissent, or contravene the policies and procedures of LSSID will face disciplinary action.
Bullying and unkindness, whether physical or emotional, are entirely unacceptable at LSSID. This type of behaviour conflicts sharply with the school's principles and we will always take it seriously. Bullying is behaviour by an individual or group, repeated over time, which intentionally hurts another individual or group, either physically or emotionally.
Bullying can take many forms (bullying doesn't only include face-to-face interactions and may include cyber-bullying via text message, snapchat, Instagram, or other types of social media). If you feel you are being bullied, or are concerned about bullying, please let your teacher know immediately.
CLASS CONDUCT EXPECTATIONS
While in class, student expectations are as follows:
- No gum or eating is permitted while in class.
- No running/horseplay/gymnastics.
- Dancers must arrive ready for their class and on time. This means wearing the appropriate attire with shoes on! Classes will begin promptly at their designated times.
- When class begins, dancers are expected to start warming up and stretching immediately.
- Dancers are expected to fully participate in exercises and class drills, and are not to leave partway through an exercise for a break, rather should finish the exercise with their classmates.
- Respect the instructor and fellow dancers.
- Use class time productively! If the instructor is not working directly with a dancer, it is expected that they make use of their time by practicing their material unless it is a designated break time. Sitting around chatting is not a productive use of time and is distracting to the instructor and the other students.

DISCIPLINARY POLICY
The goal of LSSID is to make dance class a fun, positive place that is supportive to individual and group learning, as well as striving for, and accomplishing goals. We sincerely hope that we will not ever have to enforce this policy. Our expectation is that every dancer will treat others as they would like to be treated and comply with the guidelines contained within our Dancer Conduct policy, also outlined in our Parent Information handbook.
In a situation where these expectations are not adhered to, discipline appropriate to the age group of the dancer(s) involved will be enforced. For small children, they will be asked to sit down in a short timeout. Older students will be given one warning and if an offense is repeated following a warning, they may be excused from the studio for the remainder of the class. Students who continue to disrupt the class may be suspended from class, and parental involvement will be called upon.
Disciplinary action is applicable in cases of profanity, verbal or physical altercations, or where continued disruptiveness, outbursts, bullying, disregard and/or disrespect toward an instructor or classmate is evident. Should a student commit any of these offenses during a class, or at any dance related activity, or outside of dance activities through a variety of social media channels, the following measures may be taken at the instructor's discretion:
Parent notification and student will be required to provide a verbal apology to classmates and instructor.
Parent notification and two-week suspension from class and related dance activities.
